Looks like you've ruled out some of the possibilities. Carbon frames have metal hangers but of your indexing is fine through the block that rules it out too. If you have the ability to put your bike's cassette on the trainer you should be able to answer this - assuming of course that when your bike ...

There are really maybe only three actually maybe four things it can be... Indexing Bent hanger/frame Worn cassette and or chain. Freehub body not straight So if you worked through them.... If it's in ERG mode you will always have this issue - put it into the small chain ring and mid cassette and the resistance will adjust downwards as you start - it;s trying to adjust the resistance to what you're putting in so trying to turn 250 watts say in three revs is a great deal harder than 250...

There's a thread similar to this over on the trainerroad forum about miss aligned cassette on the trainer.
